Sign up Form.
STEP
1: We
need you to provide us with an authorization
to work on your credit reports. This authorization
must be given to New West Credit Consultants
to challenge
inaccurate or negative information.

us to act on your behalf to challenge the negative
items that appear on your credit report.

Sign this document and fax the last two pages with your choice of programs
